Pacific Coastal Wellness Clinic (“Clinic,” “we,” “us,” or “our”) is a licensed Oregon telehealth provider offering medical services including medical weight loss, hormone therapy, and related care to Oregon residents. This Privacy Policy describes how we collect, use, disclose, and protect your information, including your Protected Health Information (PHI) as defined under the Health Insurance Portability and Accountability Act (HIPAA).

2. Information We Collect

  • Personal identifiers: name, email address, phone number, date of birth, and mailing address.
  • Health and medical information: health history, medications, diagnoses, laboratory results, treatment plans, and other information provided during the course of treatment.
  • Payment information: billing details processed through our third-party payment processor. We do not store full credit card numbers.
  • Usage data: IP address, browser type, pages visited, and other standard web analytics data collected through cookies and similar technologies.

3. Protected Health Information (PHI)

As a healthcare provider, Pacific Coastal Wellness Clinic may collect, maintain, use, and disclose Protected Health Information (PHI) in connection with the medical services we provide. PHI may include medical history, medications, diagnoses, laboratory results, treatment plans, communications related to patient care, and other health information provided during the course of treatment.

Pacific Coastal Wellness Clinic uses and discloses PHI only as permitted by applicable federal and state laws, including for purposes of treatment, payment, healthcare operations, and other uses authorized by the patient or required by law.

PHI may be shared with pharmacies, laboratories, healthcare providers, and service providers involved in a patient’s care when necessary to provide treatment, coordinate care, process prescriptions, obtain laboratory testing, or operate the practice.

Pacific Coastal Wellness Clinic maintains administrative, technical, and physical safeguards designed to protect PHI from unauthorized access, use, or disclosure.

4. Your HIPAA Rights

  • Right to access: You have the right to inspect and obtain a copy of your PHI held by the Clinic. We will respond to access requests within 30 days.
  • Right to amend: You may request that we correct or amend inaccurate or incomplete PHI.
  • Right to an accounting of disclosures: You may request a list of certain disclosures of your PHI made in the past six years.
  • Right to request restrictions: You may ask us to restrict how we use or disclose your PHI.
  • Right to confidential communications: You may request that we communicate with you in a specific way or at a specific location.
  • Right to file a complaint: If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr. We will not retaliate against you for filing a complaint.

5. Website Data

Non-health information collected through our website (such as usage and analytics data) is used solely to operate and improve our online services. We do not sell personal information of any kind to third parties.

6. Third-Party Service Providers

Vendors who access PHI on our behalf are required to sign a Business Associate Agreement (BAA) and are contractually obligated to protect your information in accordance with HIPAA. These vendors may include electronic health record systems, telehealth platforms, pharmacies, laboratories, and payment processors.

7. Data Retention

We retain medical records and PHI for a minimum of 10 years from the date of service, or as required by Oregon state law, whichever is longer.

8. Changes to This Policy

We reserve the right to update this Privacy Policy at any time. Material changes will be posted on this page with an updated effective date. If changes affect how we use your PHI, we will provide notice as required by HIPAA.
